MENTEE APPLICATION FORM
MENTEE ELIGIBILITY CRITERIA:
- 1 year of experience working at Griffin
- Part-time or full-time employee
- Complete application & submit 2 professional references
- Must be able to commit to the following:
- Participate in mentee training program & orientation
- 1:1 monthly meeting with mentor at minimum for 6-month duration
- Complete feedback survey

MENTOR APPLICATION FORM
MENTOR ELIGIBILITY CRITERIA:
- 1 year of experience working at Griffin
- 5 years of work experience
- Full-time employee
- Complete application & submit 1 professional reference
- Must be able to commit to the following:
- Participate in mentor training program & orientation
- 1:1 monthly meeting with mentee at minimum for 6-month duration
- Participate in monthly mentor check-ins for 6-month duration
